Order Form 1 - Use this order form if you would like to fill it out with a pen. This is also the form to use if you intend to fill it out later when your offline.

Order Form 2 - Use this editable order form while online and print a copy to send in with your cup. You can fill out now and print a copy if your ready to send in your cup now.

Here's how it works, ship your cup(s) to the address on the order form. When we receive your cup(s) we will, at your direction, cut the cup in one of several ways: with the handle on the right side, left side, center, or with no handle at all.

You can also use our second option of requesting a particular style and pattern of cup, either by phone or email. Provide as much information as possible, a picture will help tremendously if this is your own china pattern that you are currently using. We will search for your particular pattern and make your night light especially for you. Keep in mind, the cost will be the current going rate for that particular style and pattern of cup.

** Disclaimer: We make no guarantee that we will actually be able to make your cup into a night light. If we are able to cut your cup in half (we have a 2% breakage rate) then we will turn it into a night light and ship it to you as listed above. If for some reason your cup is broken during the cutting process, we will refund your money and return your broken cup to you at our cost. We choose to return the cup to promote our honesty and integrity. If your cup is valuable to you and this is not satisfactory, then we suggest you contact Rick and request your cup style and pattern as described with the second option. (return)
